We accept BCBS of Michigan plans,  Ascension Health, ABS Bronson, and many out of state BCBS plans.  

We accept Medicare, Medicare + Blue Cross Blue Shield (Med + Blue), Aetna Medicare, and UHC Medicare Advantage.  We do NOT accept Medicare/Medicaid plans such as Meridian, Meritain, or Wellcare.

We accept UHC, UMR-UHC, UHOne. We do not accept UHC Dual Complete.

We are not in network with Humana or Humana Medicare, but will accept your out of network benefits.

VACCN- Patient must intitiate VA referral and authorization themselves or have the referring MD's office initiate and have auth sent to our clinic in order to use VACCN insurance. Battle Creek VA ph # 269-934-9123

We do NOT accept Medicaid, Medicare/Medicaid Plans (with an exception for QMB), UHC Dual Complete or any plan that has Medicaid as partial payer.

PRIVATE PAY RATE

We accept private pay for individuals who have insurance we do not accept at a rate of $125 per visit.
