Core Balance Patient Forms

We will send digital Initial Evaluation forms via email or text prior to your first appointment.  If you do not receive them, or you would prefer to print and complete the forms, please download the NEW PATIENT INTIAL EVALUATION forms and fill them out before your appointment.  Bring the completed forms, your insurance cards, picture ID,  and your referral (if we do not already have it) to your initial evaluation appointment.  Please arrive 10 minutes before your scheduled appointment time.

If you would like to release your physical therapy records to yourself or one of your providers, please download and complete the Medical Release Form.  Bring it to our office or mail it to 2026 Parkview Ave., Kalamazoo, MI 49008.
